КАДРОВЫЙ ДЕФИЦИТ В ЗДРАВООХРАНЕНИИ: ПРИЧИНЫ И ПУТИ ПРЕОДОЛЕНИЯ
Аннотация
В российском здравоохранении резко обострилась проблема дефицита кадров. Цель статьи – исследовать причины и возможные пути ее преодоления. Рассматриваются вопросы о недостаточности текущих мер кадровой политики государства, о требуемых дополнительных мерах и об их ожидаемом эффекте. Анализируются проекты нормативных документов, отечественная и зарубежная литература, текущая периодика по этому вопросу. Анализ показывает, что проблема не сводится к оттоку кадров из государственного здравоохранения: корни кадрового дефицита уходят глубоко в сложившуюся систему организации медицинской помощи и государственного управления. Исследуются главные факторы возникшего дефицита кадров: чрезмерная специализация врачебной деятельности в первичной медико-санитарной помощи, несовершенство структурной и инвестиционной политики в отрасли, серьезные дефекты в планировании кадрового обеспечения и в регулировании подготовки врачей, недостаточность мер социальной поддержки медиков. Возможности экстенсивного развития многих секторов здравоохранения уже исчерпаны. Масштабное строительство, включение в государственные гарантии новых видов услуг не укладываются в кадровые возможности отрасли. Текущие меры кадровой политики недостаточны для преодоления кадрового дефицита, поскольку они не затрагивают первопричин проблемы. Развитие долговременного планирования и усиленное государственное регулирование структуры врачебных специальностей способно сфокусировать подготовку медиков на ликвидацию нехватки самых востребованных кадров, причем в ближайшие годы. Преодоление чрезмерной специализации врачей первичного звена, изменение структуры кадров, повышение размера оплаты труда потребуют более значительного периода времени.
